- High School diploma or equivalency diploma (Must submit copy with application).
- Pass the Diagnostic Entrance Test offered by the Nursing program (See Pre-Entrance Test Letter and Dates and call to register for the test).
- Physical examination by healthcare provider indicating you are in good health with no physical restrictions and able to perform the duties of a nurse.
- Required immunizations (See Vaccination Policies).
- Criminal Background Check and Clearance - Requests by health care agencies for criminal background checks and drug testing on nursing students are becoming more common. Nursing students may be required, at their own expense, to complete a child abuse background check and/or state or federal background check before beginning a clinical placement. Inability to obtain a satisfactory clearance can jeopardize a student’s ability to attend clinical, meet clinical outcomes, as well as register for the licensure exam.
- On the application for New York State licensure, the applicant is required to truthfully answer the following questions:
- Have you ever been found guilty after trial, or pleaded guilty, no contest, or nolo contendere to a crime (felony or misdemeanor) in any court?
- Are criminal charges pending against you in any court?
- Has any licensing or disciplinary authority refused to issue you a license or ever revoked, annulled, cancelled, accepted surrender of, suspended, placed on probation, refused to renew a professional license or certifcate held by you now or previously, or ever fined, censured, reprimanded or otherwise disciplined you?
- Are charges pending against you in any jurisdiction for any sort of professional misconduct?
- If the answer to any of the questions is yes, the applicant must offer full explanation, submit court records (conviction records) and if you possess one, a copy of the “Certificate of Relief from Disabilities or “Certificate of Good Conduct”. Once this information is submitted, the State Education Department will decide if you are eligible for licensing.
- Once you meet the above requirements, you then must
- Attend a mandatory orientation session
- Submit a $75 non-refundable commitment fee
- Interview with the Adult Counselor
- Complete a criminal background check and drug testing (Olean Practical Nursing students)
